Reavis Rehab accepts insurance plans that allow Out-Of-Network Options, at (or near) in-network rate.
Limitations Regarding Superior Health Patients: All new referrals must go through Care Options for Kids (COFK), unless they are under the age of “3” , over age of “21”, are STAR KIDS. Patients can OPT OUT after they have their Evaluation and go to any Superior provider.
